We are required to follow state regulations regarding vaccinations required for school entry in Vermont. Please provide  an updated copy of your child’s immunization record each year.

If you are unable to obtain a copy of your child’s immunization record from their primary care provider, you can request a copy directly from the Vermont Immunization Registry.

Vermont allows vaccination exemptions or religious reasons or for medical reasons. To qualify for an exemption, you must submit a new exemption request form annually. Contact your school nurse with any questions.
